RESUMO

Objective:To investigate various clinical treatment measures for thoracolumbar fractures and provide new diagnosis and treatment methods.Methods:The case data of 5 patients(3 males, 2 females, and the age ranged from 39 to 59 years with an average of 51 years) with thoracolumbar fractures were treated with Waveflex semi-rigid internaI fixation system from May 2020 to December 2021 were retrospectively analyzed, imaging indexes and clinical effects were followed up, and analyzed and summarized in combination with relevant literatures.Results:The operations of all 5 patients were successfully completed, and the follow-up period was 6 months. At the last follow-up, the internal fixation position was good, the height of the injured vertebra was maintained satisfactorily, and the clinical effect was satisfactory.Conclusion:Waveflex semi-rigid internal fixation system combines the advantages of fusion and non-fusion, providing a new idea for the treatment of thoracolumbar fractures, but its long-term effect still needs further follow-up.

RESUMO

Non-fusion technology for lumbar degenerative disease is one of the hotspots in current spinal surgery research. The advantage is that it can maintain the stability of the lumbar spine and retain a certain degree of lumbar spine movement. Commonly used non-fusion technologies include artificial intervertebral disc replacement, pedicle screw systems, interspinous fixation systems, and artificial facet arthroplasty replacement systems. This article will introduce the recent development of the above four types of technologies.

Objective The aim of this study was to investigate the expression of WEE1 in gastric cancer and its influence on prognosis of patients. Methods Seventy-eight patients with gastric cancer were enrolled. According to the presence or absence of lymph node(LN)metastasis,the patients were divided into LN( -)group and LN( +)group. The expression of WEE1 in gastric canc-er tissues was detected by immunohistochemistry and RT-qRCR techniques. The influence of WEE1 expression on LN metastasis and prognosis of gastric cancer were analyzed statistically. Results (1)The positive rate of WEE1 in gastric cancer was 43. 6% ,of which 13 cases were a low expression and 21 cases with high expression. The positive rates of WEE1 in LN( +) group and LN( -) group were 53. 8% and 23. 1% ,respectively. (2)The results of RT-qPCR showed that the mRNA expression of WEE1 in LN( -)and LN ( +)groups was(1. 32 ± 0. 21) and(3. 64 ± 0. 41),respectively,and the difference was statistically significant(P<0. 01). (3) The results of ROC curve showed that the area under curve(AUC)of WEE1 mRNA expression level for LN metastasis of gastric cancer was 0. 806,the sensitivity was 84. 8% ,the specificity was 79. 6% ,and the diagnostic efficacy was good. T stage and LN metastasis were independent risk factors for the expression of WEE1(P<0. 05). (4)Among 78 patients,7 patients were lost to follow-up,the rate of loss of follow-up was 8. 9% ,and 41 patients died within 5 years. Among them,13,10 and 18 died in the WEE1( -)group,low-ex-pression of WEE1 group and high-expression of WEE1 group,respectively. The 5-year survival rates of the three groups were signif-icantly different(χ2 =25. 67,P<0. 001). The survival rate in the low expression of WEE1 group was not significantly different from the high expression of WEE1 group(P>0. 05),but both of them were significantly low the WEE1( -)group(P<0. 05). Conclusion WEE1 has a high positive rate in gastric cancer,and is closely related to patient stage and LN metastasis. The positive expression of WEE1 is a strong signal for poor prognosis.

Objective To explore the expressions and clinical values of pro-gastrin-releasing peptide(ProGRP) and carbohydrate antigen 72-4 (CA72-4) in patients with gastric cancer.Methods Ninety patients with gastric cancer and fifty healthy subjects were selected from January 2014 to December 2016 in our hospital.Serum levels of ProGRP and CA72-4 were detected by electrochemiluminescence.The relationships between ProGRP and clinicopathological characteristics,postoperative recurrence and CA72-4 were analyzed.The diagnostic values of ProGRP and CA72-4 in gastric cancer were analyzed by receiver operating characteristic (ROC) curve.Results The expressions of ProGRP and CA72-4 in patients with gastric cancer were (249.3 + 28.9) pg/ml and (148.8 + 33.5) U/ml respectively,which were significantly higher than those of healthy subjects [(14.4 ± 7.6) pg/ml and (3.8 ± 1.4) U/ml],and the differences were statistically sigificant (t =56.320,P < 0.001;t =30.504,P < 0.001).The expression of ProGRP in TNM stage Ⅲ-Ⅳ [(269.1 ±30.9)pg/ml] was obviously higher than that in stage Ⅰ-Ⅱ [(198.5 +23.9)pg/ml],with a significant difference (t =11.200,P < 0.001).The expression of ProGRP in patients with lymph node metastasis [(259.9 ±31.4)pg/ml] was significantly higher than that in patients without lymph node metastasis [(190.3 ±26.8)pg/ml],with a significant difference (t =9.500,P < 0.001).The expression of ProGRP in patients with postoperative recurrence after one year [(181.3 ± 21.7)pg/ml] was higer than that in patients without postoperative recurrence [(26.1 ± 12.8)pg/ml],with a significant difference (t =31.830,P < 0.001).There was a positive correlation between serum ProGRP and CA72-4 (r =0.792,P =0.012).According to the ROC curve,the cut-off point of ProGRP was 23.6 pg/ml,and the diagnostic sensitivity was 80.0%,the specificity was 70.0%.The cut-off point of CA72-4 was 11.2 U/ml,and the diagnostic sensitivity was 60.0%,the specificity was 89.0%.The sensitivity and specificity diagnostic value of combined detection were 89.7% and 94.8%,better than those of individual detection (x2 =6.028,P =0.009;x2 =4.675,P =0.031).Conclusion ProGRP and CA72-4 are highly expressed in the serum of gastric cancer patients,with a positive correlation.The combined detection of ProGRP and CA72-4 can improve the diagnostic sensitivity and specificity.ProGRP is significantly correlated with tumor stage,lymph node metastasis and prognosis,which may be a mew target for prevention and treatment of gastric cancer.

Objective To evaluate choroidal thickness (CT) in healthy and primary open-angle glaucoma (POAG) eyes using deep-range imaging optical coherence tomography (DRI-OCT) and to analyze the relationship between choroidal thickness and glaucoma.Methods Totally 140 patients (216 eyes) with POAG were selected as glaucoma group,and 67 patients (106 eyes) with normal eyes as normal group.The optical coherence tomography produced by Japanese company (Topcon) was used to detect CT in both groups.The association between CT and potential confounding variables including age,axial length,central corneal thickness,systolic pressure,diastolic pressure,systolic perfusion pressure,diastolic perfusion pressure,intraocular pressure,and visual field defects were examined by using univariable and multivariable regression analyses.Results The age,axial length,systolic blood pressure and systolic blood pressure of the glaucoma group were all higher than those of the normal group (all P <0.05),and the subjects in the glaucoma group had more severe visual field damage,and the difference of visual field defect was statistically significant (P < 0.001).There was no significant difference in corneal thickness,diastolic blood pressure,intraocular pressure and diastolic perfusion pressure between the two groups (all P > 0.05).The CT was (157.70 ± 48.54) μm in the glaucoma group,and (179.9 ± 36.1) μm in the normal group,respectively,and the difference was statistically significant (P < 0.001).Uninitiated regression analysis showed that CT was negatively correlated with age,axial length and glaucoma (all P < 0.05).Multivariate regression analysis showed that there was no significant difference in CT between glaucoma group and normal group when the length of the age and the axis of the eye were corrected (P =0.216),and moreover,there was also no association between glaucoma severity and CT.Conclusion Glaucoma is not associated with CT measured using DRI-OCT;however,older age and longer axial length were associated with thinner CT,so CT cannot be used as a marker for diagnosing POAG and assessing the condition of the disease.

RESUMO

There is no doubt that the traditional Chinese medicine(TCM) is effective, practical and scientific after it was used for thousands of years. However, the mechanisms of action of many TCM are still unclear because of their multi-component, multi-target and multi-level features, which hinder the modernization and internationalization of the TCM. Proteomics is to analyze the composition and activity of intracellular proteins which are changing dynamically from a holistic perspective. It is consistent with the holistic and dynamic views of the TCM and brings about the hope of clarifying the mechanism of action of the TCM. In recent years, great progress has been made in the application of proteomics to determine the mechanism of the TCM. This article introduced the core technologies of proteomics and systematically summarized the applications of proteomics in the study of the mechanism of the Chinese medicinal formulae, single Chinese medicine and monomeric compounds from the TCM to provide innovative ideas and methods for reference.

Objective To develop an organization and commanding simulated training system for nuclear,biological and chemical (NBC) medical rescue.Methods System engineering,expert consulting,software engineering and etc were used to develop the system with considerations on the present situation of NBC medical rescue,organization and commanding as well as the requirements of the organization and commanding simulated training.The system was composed of modules for scenario establishment,direction monitoring,command practice,program direction,performance assessment,basic database and etc.The seat settings for maneuvers of medical rescue were proposed as well,and the flows of information and rear services in maneuvers were summarized and analyzed.Results The system developed in the present research was utilized in the visualized and dynamic training of medical rescue commanders.Conclusion The system developed gains advantages in safety,efficiency-cost ratio and evaluation,and contributes to enhancing the organization and commanding of NBC medical rescue.

AIM To observe the effects of Sangtongjian Mixture (STJ) on glucose and lipid metabolism,insulin resistance and fat cytokines in type 2 diabetic rats,and their mechanisms of action.METHODS One hundred and forty rats fed on the combination of STZ and high fat diet were established as the type 2 diabetic models.Fasting blood glucose (FBG) level reached more than 16.7 mmol/L and then the rats were randomly divided into model group,metformin (180 mg/kg) group,STJ (73.5,147 and 294 mg/kg) groups.Ten rats were set as the blank group.Each treatment group was intragastrically given the corresponding agents for twelve weeks.The fasting blood glucose levels of rats were measured once every two weeks after the administration.After a 12-week administration period,glycosylated serum protein (GSP),glycosylated hemoglobin (GHb) and lipid profile indices (TC,TG,HDL-C and LDL-C) were determined.The serum insulin level was measured by radioimmunoassay,and homeostasis model assessment of insulin resistance (HOMA-IR) and insulin sensitivity index (ISI) were calculated.The levels of serum adiponectin and leptin were detected by ELISA.RESULTS STJ remarkably decreased the levels of FBG,GSP,GHb,TC,TG,LDL-C,leptin and HOMR-IR in type 2 diabetic rats.Furthermore,STJ also significantly increased the levels of HDL-C,adiponectin and ISI.CONCLUSION STJ can improve glucose and lipid metabolism in type 2 diabetic rats by ameliorating insulin resistance and regulating fat cytokine levels.

Diabetes mellitus is a characterized by high blood sugar metabolic disease, is a lifelong disease with a high incidence of major hazards. Prevention and treatment of diabetes and its complications has become a serious challenge and arduous task facing the world pharmaceutical researchers. Oxidative stress, Nfr2-NF-κB signaling axis related epigenomic genes have the apparent close relationship with type 2 diabetes,those have become one of the key focus and effective way to explore its pathogenesis, mechanism and drug screening. This paper systematically summarizes the current stage research regulating the key proteins, mRNA about Nrf2-NF-κB axis pathway and epigenomics for treatment of type 2 diabetes and the mechanism of traditional Chinese medicine and natural medicine (component) in the treatment of type 2 diabetes, hoping to provide some innovative research ideas for finding new drugs of the treatment of diabetes from traditional Chinese medicine and natural medicine.

Objective To investigate effect of alendronate on prevention of aseptic loosening prothesis.Method Twelve beagles which had been implanted a titanium plug in femur were divided into two groups randomly,one received alendronate,and the other placebo as control.After 12 weeks,all beagles were sacrificed.specimens of femur were studied by biomechanical test.Independent sample t test was used for statistical analyses.Results The pull-out strength of the experimental group was higher than that of the control (P < 0.05),and the torsion strength was also higher in the experimental group(P < 0.05).Conclusion The therapy of alendronate can change the bone strength,inhibit aseptic loosening of prosthesis in beagle and could be a good drug for the prevention and treatment of aseptic loosening prothesis.

<p><b>OBJECTIVE</b>To observe the effect of Tongsaimai (TSM) tablets in treating foot trauma of diabetic foot (DF) model rats, and discuss its potential mechanism.</p><p><b>METHOD</b>Male SD rats were selected to duplicate the diabetic foot ulcer model and randomly divided into the blank control group, the model group, the metformin treatment group, and TSM 12.44, 6.22, 3.11 g x kg(-1) groups (n = 10). The healing of ulcer wounds were observed on day 1, 4, 8, 13 and 18. After 18 days, a histopathologic examination was conducted for ulcer tissues. The contents of superoxide dismutase (SOD) and malondialdehyde (MDA) were detected by hydroxylamine and TBA methods. The content of interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-alpha) were determined with the radioimmunoassay. The immunohistochemical method was used to observe the expression of vascular endothelial growth factor (VEGF) in ulcer tissues and the number of capillary vessels.</p><p><b>RESULT</b>TSM could alleviate the pathological changes of diabetic foot rats, accelerate the ulcer healing on 4, 8, 13, 18 d, reduce MDA, IL-6, TNF-alpha, VEGF content in rat serum at 18 d (after the rehabilitation period), and enhance the SOD content. Specifically, the TSM 12.44 g x kg(-1) group showed significant differences compared with the model group (P < 0.05, P < 0.01). At 18 d after the treatment (the late rehabilitation period), the VEGF expression of TSM 12.44, 6.22 g x kg(-1) groups and the number of blood capillaries of the TSM 12.44 g x kg(-1) group were significantly lower than that of the model group (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>TSM could promote the foot wound healing of DF model rats, reduce MDA, IL-6 and TNF-alpha levels in serum, increase the SOD content and decrease the VEGF expression and the number of blood capillaries in the late rehabilitation period. Its action mechanism may be related to the inhibition of oxidative stress injury and the inflammatory cell infiltration.</p>

<p><b>OBJECTIVE</b>To clarify drug properties of components in Euodiae Fructus.</p><p><b>METHOD</b>The rat cold syndrome model was induced by cold water stress method. The content of neurotransmitters sand hormones such as DA, 5-HT, NE, AChE and 17-OHCS in serum of model rats were taken as the indexes to evaluate drug properties of components in Euodiae Fructus.</p><p><b>RESULT</b>Euodiae Fructus and its components could correct or relief the content of energy metabolism and substance metabolism-related neurotransmitters sand hormones in serum of model rats with water-stressed cold syndrome.</p><p><b>CONCLUSION</b>Euodiae Fructus and its components are proved to show hot property.</p>

RESUMO

<p><b>OBJECTIVE</b>To investigate chemical constituents from Chinese herbal medicine Hydrangea macrophylla.</p><p><b>METHOD</b>The compounds were separated and purified by column chromatography over silica gel, ODS, and preparative HPLC. Their structures were identified by spectral methods including 1H, 13C-NMR and MS.</p><p><b>RESULT</b>Eleven compounds were isolated and identified as zeorin, hopane-6, 22-diol (1), botulin (2), betulinic acid (3), 2-ethyl-3-methyl-maleimide-N-beta-D-glucopyranoside (4), uridine (5), thymidine (6), adenosine (7), nicotinamide (8), methyl pyroglutamate (9), hydrangenol (10) and hydrangenol-4'-O-beta-D-glucopyranoside (11), respectively.</p><p><b>CONCLUSION</b>Compounds 14 and 7-9 were obtained from the genus Hydrangea for the first time.</p>

AIM@#To study the chemical constituents of the aerial parts of Cirsium setosum (Willd.) MB..@*METHODS@#The chemical constituents were isolated and purified by various chromatographic techniques. Their structures were determined on the basis of physical properties and spectroscopic data.@*RESULTS@#A new megastigmane glycoside and six known compounds were obtained and identified as (7E, 9R)-9-hydroxy-5, 7-megastigmadien-4-one-9-O-α-L-arabinopyranosyl-(1→6)-β-D-glucopyanoside (1), (6R, 7E, 9R)-9-hydroxy-4, 7-megastigmadien-3-one-9-O-α-L-arabinopyranosyl-(1→6)-β-D-glucopyranoside (2), urolignoside (3), 4, 9, 9'-trihydroxy-3, 3'-dimethoxy-8-O-4$\prime $-neolignan-7-O-β-D-glucopyranoside (4), citroside A (5), salidrosidin (6), and adenosine (7).@*CONCLUSION@#Compound 1 is a new megastigmane glycoside, named as Xiaojiglycoside A.

RESUMO

The questionnaire and the result of 2010' Hwato Cup National University Students' Acupuncture Manipulation Skills Competition were analyzed in this paper. It was showed that the competition achieved the significant effects of enhancing acupuncture manipulation skills for the university students and promoting the standardization on teaching acupuncture manipulation skills. The teachers and students were not very satisfied with the current acupuncture manipulation skill education. In the competition, only the item of reciting the classics achieved more than 90 scores, which just displayed the memory ability. The results of the manipulation competition were generally not very high. It is suggested that concerning to teaching acupuncture and moxibustion in the future, the ratio of class time for improving the practice ability on the human body should be increased and the practice on some manipulation techniques such as inserting the needle by holding the needle tip with the pressing hand, reinforcing-reducing technique by twirling and rotating the needle and warming needle technique should be intensified. It is necessary to enhance the interpretation, emphasis and supplementation on the keys and details of some manipulations of acupuncture and moxibustion.

<p><b>OBJECTIVE</b>To analyze the risk factors for hospital mortality after operations for type A aortic dissection.</p><p><b>METHODS</b>Totally 766 consecutive patients (586 male and 180 female patient, aged (45±12) years, ranging from 16 to 78 years), who underwent surgery for type A aortic dissection from January 2001 to December 2010, were studied retrospectively. Preoperative and operation related clinic factors were analyzed by univariate analysis, followed by Logistic regression model, to identify the risk factors of hospital mortality.</p><p><b>RESULTS</b>Overall, 37 patients (4.8%) died during hospitalization. On univariate analysis, significant risk factors for hospital mortality were male, acute status, renal dysfunction, cardiac dysfunction, cardiopulmonary bypass time, duration of operation, volume of blood transfusion, re-operation for bleeding (χ2=4.008-27.093, P<0.05). On Logistic regression model, independent risk factors were acute status (OR=2.784, 95%CI: 1.166-6.649, P=0.021), renal dysfunction (OR=6.285, 95%CI: 1.738 - 22.723, P=0.005), cardiac dysfunction (OR=3.052, 95%CI: 1.083-8.606, P=0.035), re-operation for bleeding (OR=3.690, 95%CI: 1.262-10.791, P=0.017), volume of blood transfusion (OR=1.033, 95%CI: 1.008-1.058, P=0.010). Additionally, male (OR=0.387, 95%CI: 0.177-0.848, P=0.018) was protective factor, and alternatively, female was indeed one of the independent risk factors for hospital mortality.</p><p><b>CONCLUSION</b>Female, acute status, renal dysfunction, cardiac dysfunction, re-operation for bleeding, volume of blood transfusion were independent risk factors for hospital mortality after operations for type A aortic dissection.</p>

Objective To evaluate the value of the physiological and operative severity score for the enumeration of mortality and morbidity (POSSUM) and P-POSSUM in predicting the risks of orthopedic surgeries for senile patients with femoral neck fracture.Methods A total of 108 patients with femoral neck fractures who underwent hip joint replacement were retrospectively studied using POSSUM and P-POSSUM scoring system to predict their mortality and complication rate.The difference between predictive value and observed value was analyzed by chi-square test.Meanwhile,the patients were divided into two groups based on their POSSUM scores.The differences between two groups were analyzed.Results According to POSSUM scores,47 patients were predicted to have complications(the mean rate was 43.52%),but only 37 did actually (the rate was 34.26%).There was no significant difference between predicted values and observed values (P =0.238).The predicted death toll was 11 cases (the mean rate was 10.19%),but actually only 2 patients died (the rate was 1.85%).Predicted value was higher than observed value.In terms of complications,death toll agreed well with the predicted values calculated by P-POSSUM (predicted death of 4 cases' the mean mortality being 3.70% ; actual death of 2 cases' the mortality was 1.85%) without significant difference (P =0.625).We divided the patients into two groups with the POSSUM scores 40,and there was no significant difference between predicted values and observed values (P =0.527,P =0.285).Conclusions POSSUM has better predictive ability of morbidity,but overestimates mortality.P-POSSUM more accurately predicts mortality than POSSUM.The predicted results of POSSUM and P-POSSUM scoring systems are satisfactory in the high risk group.

RESUMO

<p><b>OBJECTIVE</b>To study the expression of connective tissue growth factor (CTGF) and its significance in sporadic ascending thoracic aortic aneurysm (AAA), and initially to investigate the mechanisms of pathological remodeling in AAA.</p><p><b>METHODS</b>AAA specimens were taken from 18 patients during elective surgical intervention, and 18 control specimens of ascending aorta were obtained from patients undergoing coronary artery bypass surgery. Specimens were stained with HE and Masson to evaluate the arrangement and aggregation of cells and collagen types I and III; immunohistochemistry staining was performed using antibodies directed against markers of CTGF; real-time PCR analysis was performed to quantify the expression level of CTGF and collagen types I and III.</p><p><b>RESULTS</b>Pathological results show degradation of elastin and hyperplasia of collagen fibers as well as disordered arrangement of smooth muscle cells in AAA. When compared with controls, protein levels of CTGF were significantly increased [(44 ± 4)% vs. (33 ± 5)%, P < 0.01]. Similar patterns were shown in mRNA levels of CTGF (P < 0.01). Using real-time PCR method, elevated levels (relative expression ratio of mRNA: 10.54/3.8 and 1.79/1.19, respectively; P < 0.01, both) of collagen types I and III were observed. CTGF expression had a correlation with both collagen fibers and aortic aneurysm diameter (r = 0.784, P < 0.01; r = 0.793, P < 0.01).</p><p><b>CONCLUSIONS</b>These results indicate increased expression of aortic collagen types I and III as well as CTGF in AAA specimens, which is likely to be responsible for the aortic wall pathological remodeling. The expression of CTGF was positively correlated with the aortic diameter. As a cytokines factor can stimulate collagen synthesis, CTGF may be involved in the pathogenesis and progression of AAA.</p>
